29 CFR 1910.217(f)(2): Operator(s) were not trained and instructed in the safe method of work before starting work on mechanical power press(es):(a) a) On or about August 24, 2016, the employer did not ensure that employees were trained in the safe operation of mechanical power presses such as, but not limited to, #4 and # 31. Training was not provided regarding the hazards associated with the key used to control the mode selection and lock out tag out during die changes and set up. Operators were exposed to amputation injuries.
