On or about August 9th 2016 in the Multi-Purpose Unit at Arkema, Inc. located in Crosby, TX, the frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience. Equipment includes but is not limited to: a) Sulfur Dioxide T-19 (P&ID CR 17-X-0017 Rev.
