29 CFR 1926.850(h): When debris is dropped through holes in the floor without the use of chutes, the area onto which the material is dropped shall be completely enclosed with barricades not less than 42 inches high and not less than 6 feet back from the projected edge of the opening above. Signs, warning of the hazard of falling materials, shall be posted at each level. Removal shall not be permitted in this lower area until debris handling ceases above. a. On or about August 4, 2016, located at the northeastern portion of the 2nd floor at the former Columbus Municipal Light Plant, employees were removing scrap material by throwing the material off of an open-sided floor into the basement of the building. There were no barricades and warning signs where the material landed, thereby exposing the employees on the lower floor to a struck-by hazard. b. On or about August 4, 2016, located at the northwestern portion of the 2nd floor at the former Columbus Municipal Light Plant, employees were removing scrap material by throwing the material off of an open-sided floor into the basement of the building. There were no barricades and warning signs where the material landed, thereby exposing the employees on the lower floor to a struck-by hazard. c. On or about August 4, 2016, located at the northeastern portion of the 3rd floor at the former Columbus Municipal Light Plant, employees were removing scrap material by throwing the material off of an open-sided floor into the basement of the building. There were no barricades and warning signs where the material landed, thereby exposing the employees to a struck-by hazard. d. On or about August 4, 2016, located on at the southeastern portion on the 3rd floor at the former Columbus Municipal Light Plant, employees dropped scrap material through a floor hole to the basement. The area where the material was being dropped was not enclosed with barricades and warning signs, thereby exposing the employees to a struck-by hazard. e. On or about August 4, 2016, located at the southwestern portion on the 3rd floor at the former Columbus Municipal Light Plant employees dropped scrap material through a floor hole that was approximately 11 feet by 11 feet to the basement. The area where the material was being dropped was not enclosed with barricades and did not have warning signs, thereby exposing the employees to a struck-by hazard. f. On or about August 4, 2016, located on the 5th floor at the former Columbus Municipal Light Plant, employee dropped scrap material through a 12 foot diameter floor hole to the 3rd floor. The 3rd floor did not have any barricades and signs to prevent and warn employees from being struck-by falling material.
