29 CFR 1910.219(c)(4): Projecting shaft ends did not; (i) present a smooth edge and end or project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. (ii) Unused keyways were not filled up or covered. (a) Throughout the facility - On or about July 12, 2016, a rotating shaft on a gearbox had a ring with an inside keyway that was not filled in to present a smooth edge. A rotating shaft on a gearbox in another department had a split collar over the shaft that did not present a smooth edge at the two splits. Another gearbox has an exposed key on the inside of the collar.
