29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: On/around June 7, 2016 the employer at the Tyson Foods facility in Hope, Arkansas did not ensure that cooling fans in areas where operators enter to monitor and control equipment in the 2nd Process refrigeration system have blade guards. a) The personal cooling fan used by operators in the control room/operator office was in operation without a blade guard. b) The cooling fan for the water pump motor on the condenser unit was in operation with the blade unguarded.
