29 CFR 1910.146(i)(4): The employer did not ensure that each attendant remained outside the permit space during entry apparitions until relieved by another attendant: When the employer's permit entry program allows attendant entry for rescue, attendants may enter a permit space to attempt a rescue if they have been trained and equipped for rescue operations as required by 29 CFR 1910.146(k)(1) and if they have been relieved: On March 23, 2016, and times prior and thereto, at tent #1, rear of the facility; employees conducted permit required confined space without an attendant, exposing employees to struck by hazards.
