29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about May 20, 2015, employees were exposed to struck-by, burn, and amputation hazards when: a. The guards on the Ameripak 60 packaging machine used in the IQF2 room did not prevent an employee from placing his/her hands into the cutting/sealing mechanism while the machine was running. Employees reached into the cutting/sealing part of the machine in order to retrieve fallen product, remove incorrectly sealed product and feed plastic pouch material. An amputation took place on this machine on May 15, 2015. The employer has removed this machine from service. b. Scale station in the Freezer 1 room: When the bags do not stay in place, the employees are required to manually hold the bag so that the pre-weighed product drops into the bag. This puts the employee hand in close proximity to the trap door mechanism at the bottom of the weigh bin. When the trap door activates, an employees hand can be caught in the mechanism. c. BandRite Band Sealer in the scale area: There are no guards on the slitter blades. The employees hold onto the bags while they are going through the slitter blades until the top is cut off so that they can discard the top in the nearby trash can.
