29 CFR 1910.146(k)(3) When an authorized entrant entered a permit space, retrieval systems or methods were not used to facilitate non-entry rescue: On or about April 3, 2015, at VCNA Prairie, Inc., an employee entered the mixer dump hopper in the batch plant without the use of a retrieval system or method to facilitate non-entry rescue. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
