29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: 1.) On or about April 15, 2015, the employer did not assure each employee that was required to enter a permit required confined space was trained with the knowledge and skills necessary to safely perform entry of a permit required confined space. Employees were not trained in required permit documentation measures, that atmospheric testing was needed when entering and while in the parts washer, the need to have an attendant outside the space to help the entrant in an emergency situation, and employees were not trained on rescue procedures in the event of an emergency. Employees are exposed to caught in hazards from the chain conveyor, chemical hazards from chemicals such as but not limited to; Potassium Hydroxide and Phosphoric acid and water engulfment.
