29 CFR 1910.217(e)(1): Inspection and maintenance records. The employer shall establish and follow an inspection program having a general component and a directed component. a) On or about April 15, 2015, the employer failed to establish and follow a program of periodic and regular inspections for the AIDA 1 mechanical power to ensure that all parts, auxiliary equipment, and safeguards of the press were in a safe operating condition and adjustment. The AIDA 1 had deficiencies such as, but not limited to, inadequate rear guarding of the point of operation, the selector key not being supervised and employees not using die blocks during die changes, exposing employees to amputation injuries when operating the press. b) On or about April 15, 2015, the employer failed to establish and follow a program of periodic and regular inspections for the AIDA 2 mechanical power to ensure that all parts, auxiliary equipment, and safeguards of the press were in a safe operating condition and adjustment. The AIDA 2 had deficiencies such as, but not limited to, inadequate rear guarding of the point of operation, the selector key not being supervised and employees not using die blocks during die changes, exposing employees to amputation injuries when operating the press. c) On or about April 15, 2015, the employer failed to establish and follow a program of periodic and regular inspections for the Niagara mechanical power to ensure that all parts, auxiliary equipment, and safeguards of the press were in a safe operating condition and adjustment. The Niagara had deficiencies such as, but not limited to, inadequate rear guarding of the point of operation and employees not using die blocks during die changes, exposing employees to amputation injuries when operating the press.
