29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii): a. On or about March 17, 2015, in the Production area, at the #79 Bartell twelve-wire, six-inch reel wire stranding machine, the wire spool take up reel located at the end of the machine had two six-inch gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards. b. On or about March 17, 2015, in the Production area, at the #78 Bartell twelve-wire, six-inch reel wire stranding machine, the wire spool take up reel located at the end of the machine had two six-inch gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards. c. On or about March 17, 2015, in the Production area, at the #77 National Standard Bartell twelve-wire, six-inch reel wire stranding machine, the wire spool take up reel located at the end of the machine had two six-inch gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards. d. On or about March 17, 2015, in the Production area, at the #76 Bartell twelve-wire, six-inch reel wire stranding machine, the wire spool take up reel located at the end of the machine had two six-inch gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards. e. On or about March 17, 2015, in the Production area, at the #74 Watson twelve-wire, six-inch reel wire stranding machine, the wire spool take up reel located at the end of the machine had two gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards. f. On or about March 17, 2015, in the Production area, at the #102, #105, #106, #107, #117, #118, #110, #112, #114, #115, #116, #108 and #109 six-inch reel wire stranding machines, the wire spool take up reel located at the end of the machines had gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards. g. On or about March 17, 2015, in the Production area, at the #2 and #3 Re-reel machines, the wire spool take up reel located at the end of the machine had two gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards. h. On or about March 17, 2015, in the Production area, at the #4 Re-reel machine, the wire spool take up reel located at the end of the machine had three gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards. i. On or about March 17, 2015, in the Production area, at the #5 Re-reel machine, the wire spool take up reel located at the end of the machine had two gears that were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii) and exposed employees to caught-in hazards.
