29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. On or about March 3, 2015, at the packaging area, the employer had not developed a lockout tagout program with specific energy control procedures for the control of hazardous energy for the IQF Tunnel Freezer, Triangle Vertical Form Fill Seal Bagger, and Triangle In Line Weighers (Scale), among other machinery with potential to be energized, had not performed employee training on the control of hazardous energy and executed periodic inspections to prevent an amputation or caught-in hazard. b. On or about March 3, 2015, at the engine room, the employer had not developed a lockout tagout program with specific energy control procedures for the control of hazardous energy for the Vilter Reciprocating Compressors and Frick Rotary Screw Compressors, among other ammonia refrigeration equipment with potential to be energized, performed employee training on the control of hazardous energy and executed periodic inspections, exposing employees to asphyxiation and fire hazards.
