29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process: a) On or about December 15, 2014, P&ID ACS9912 incorrectly indicated Valve #1377 was located on the 3/4" HGD Drain Line. Valve #1377 is located on the 2" HGD Line. b) On or about January 2, 2015, P&ID ACS9912 incorrectly indicated the 2 1/2" LTRL line connects with 3/4" line containing Valves #1164 and #1155. The 2 1/2 " LTRL line and the 3/4" line containing Valves #1164 and #1155 do not connect. c) On or about January 7, 2015, P&ID ACS9912 incorrectly indicated a line with Valve #1152 connecting the ACC 10 Sight Column to the 3/4" pipe connecting V-15 Oil Pot to Pressure Relief Valve #1175. The line containing Valve #1152 also contains two T-junctions with 3/4" LTS lines and Valve #1528.
