29 CFR 1910.119(e)(3)(iv): The process hazard analysis did not address the consequences of failure of engineering controls: a) On or about 12/04/14 in the phosgene container supply room; employer failed to protect employees from exposure to phosgene when the process hazard analysis conducted on 8/15/13 did not address the consequences of having an insufficient amount of caustic in the emergency scrubber system during a catastrophic release of phosgene in the container supply room. ABATMENT DOCUMENTATION REQUIRED
