On or about March 4, 2019, at the tank battery, employee training did not include the methods and observations to detect the presence or release of crude oil from oil and gas production water at tank and pipe openings where employees worked to install ventilation piping and which exposed employees to hazardous chemicals including but not limited to crude oil (flammable liquid).
