29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: a) On or about October 29, 2011, and at times prior, Continental Resources, Inc., as the controlling employer, did not ensure that Cyclone Drilling, Inc., a subcontractor, protected their employees from injury in that, the switch box for the diesel pump supplying the mud tanks was missing a cover.
