29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment. (a) 158 West Jackson Street, New Holland PA, Lower Engine Room - On or about June 13, 2022, the knock out plug was missing from the left side side of the Engine 1 feed from Tower Distro 1 panel. Employees were exposed to an electrical hazard. (b) 158 West Jackson Street, New Holland PA, Electrical Storage Room - On or about June 14, 2022, the 240 volt disconnect housing next to the MC 11-9 disconnect was not effectively closed. The opening was wrapped with electrical tape. Employees were exposed to an electrical hazard. (c) 158 West Jackson Street, New Holland PA, Production Maintenance Room - On or about June 19, 2022, the welder disconnect housing labeled Fed from B3-P1-CB 15 above the 240 disconnect housing was not effectively closed on the right side and bottom of the housing. Employees were exposed to an electrical hazard. (d) 158 West Jackson Street, New Holland PA, Production Maintenance Room - On or about June 19, 2022, the 4 plug outlet and the connected junction box above ZEP parts washer were not effectively closed. The 4 plug outlet knock out plug was missing at the bottom of the outlet and the connected junction box knock out plug was missing on the right side. Employees were exposed to an electrical hazard. (e) 158 West Jackson Street, New Holland PA, Production Maintenance Room - On or about June 19, 2022, the light switch outlet combination electrical box next to breaker panel 42 B3 P1-39 was not effectively closed. 2 knock out plugs were missing on top of the outlet. Employees were exposed to an electrical hazard. (f) 158 West Jackson Street, New Holland PA, Hassia Electrical Hallway - On or about July 14, 2022, the 400 VAC electrical closet labeled "HASSIA-CP; Fed From: BP-G-75" had an exposed opening at the door lock to the left of the main switch. Employees were exposed to an electrical hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
