29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: The employer is failing to protect employees from struck-by hazards associated with abrasive wheels. This was most recently documented on or about 05/31/2019 at the Shamrock Tire & Auto Service, Inc. dba Tire Dealers Wholesale facility located at 3003 S. Broadway St. Wichita, Kansas 67216. The grinding machine was operated without an adjustable tongue or a safety guard at the top.
