29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about February 28, 2018, located in the mill room, employees were exposed to machine hazards associated with rotating parts of a wig wag rotating barrel and conveyor line rubber puller during the setup activity of restring the rubber strips. The employer failed to implement energy control application steps as the machines were not shut down or turned off to perform the work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards. b. On or about March 8, 2018, located in the Mandrel Hose "Geese" Finishing Department, an employee was exposed to machine hazards associated with the sharpening of the rotating blades. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards. c. On or about March 14, 2018, located in the Mandrel Hose "Geese" Finishing Department, an employee was exposed to machine hazards associated with moving parts and point of operation hazards during blade changes. The employer failed to implement energy control application steps as a LOTO device was not applied to the ball valve [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i), and verification of isolation [(d)(6)], were not implemented to protect to protect employees from machine servicing hazards.
