29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Options Medical Center dba Options, 1700 S. 120th St., Lafayette, CO 80026: On or before January 25, 2018, the employer did not ensure that each employee was adequately trained on chemicals in the workplace. Chemicals used by employees use include, but are not limited to Pure Bright Germicidal Ultra Bleach, isopropyl alcohol, pH Up, pH Down, House and Garden B Aqua Flakes Nutrient, and House and Garden B - Coco Nutrient. This condition exposed employees to chemical hazards. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
