29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources: Instance a. On or about September 23, 2016, an authorized employee did not verify that all energy isolation devises were located on the Top Notch ski chairlift system prior to allowing the maintenance procedures to begin. Instance b. On or about September 23, 2016, an authorized employee did not verify that all energy isolation devises were located on the Flying Yankee ski chairlift system prior to allowing the maintenance procedures to begin. Instance c. On or about September 23, 2016, an authorized employee did not verify that all energy isolation devises were located on the Summit ski chairlift system prior to allowing the maintenance procedures to begin. Instance d. On or about September 23, 2016, an authorized employee did not verify that all energy isolation devises were located on the East Borvig & West Borvig ski chairlift systems prior to allowing the maintenance procedures to begin.
