29 CFR 1910.134(d)(3)(i): The employer did not provide a respirator that was adequate to protect the health of the employee and ensure compliance with all other OSHA statutory and regulatory requirements, under routine and reasonably foreseeable emergency situations. a. MCM Precision Castings, Inc. located in Weston, Ohio: On or about November 20, 2014, an employee wearing a powered air purifying respirator while operating the sand blasting cabinet was overexposed to crystalline silica dust at 289.79 mg/m3 which exceeded the assigned protection factor of the respirator.
