29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): a. On or about September 20, 2013, an employee was working inside liquid AC tank number 3. Two locks had been applied to the AC Unloading Pump MCC disconnect in the control room. However, the two locks did not indicate the identity of the employees that applied the locks, nor did the color of the locks indicate the correct entity that was to have applied it, i.e., the locks were red and yellow, red indicated a contractor and yellow was not listed in the program.
