29 CFR 1926.1419(j): Anyone who becomes aware of a safety problem must alert the operator or signal person by giving the stop or emergency stop signal. (Note: �� 1926.1417(y) requires the operator to obey a stop or emergency stop signal). a. Located at the site, there were employees removing a W36x194 I-beam that was 60 feet, 3 inches in length from a bridge with a Bucyrus Erie 60 ton lattice boom crawler crane, model 30b, serial number 134548 that was rigged 6 feet, 4 inches off of the east side of the I-beam's center and 18 feet, 8-1/2" inches off of the west side of the I-beam's center. The employees identified it as safety problem and did not alert the operator or signal person of the safety hazard, thereby exposing the employees to a struck-by hazard from the load.
