29 CFR 1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee did not use insulated tools or handling equipment when the tools or handling equipment might have made contact with such conductors or parts: a) JBS USA, LLC at 800 North 8th Avenue, Greeley, CO 80631: On or about December 4, 2012, the employer did not ensure that employees working near energized conductors used insulated tools and handling equipment in the following instances: 1) Selected mechanics disengaged power to 480 volt motors by flipping the disconnects. They then reversed the direction of the motors by reversing the wiring on the disconnects. The older disconnects were not shielded over the energized parts. The mechanics did not use insulated tools when reversing the wiring, working within 8 inches of the energized parts. 2) Electricians stripped and cut energized 110 volt wiring without using qualified insulated tools. Conditions exposed employees to electrical shock hazards.
