29 CFR 1910.119(j)(3): The employer did not train each employee involve in maintaining the on-going mechanical integrity of process equipment in an overview of that process and its hazards and in the procedures applicable to the employee's job tasks to assure that the employee can perform the job tasks in a safe manner: At the establishment, training for process maintenance activities were not implemented. The employer did not provide training for refrigeration operating and maintenance employees related to maintenance of on-going mechanical integrity of Ammonia refrigeration system; the ammonia system equipment; the inspection, testing and preventive maintenance procedures applicable to the employee's job tasks to assure that the employee can perform the job tasks in a safe manner. Employees were exposed to catastrophic release of ammonia which result in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
