29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section: Located at 227 Grand Street, Brooklyn, NY On or about 3/22/2012 a) Toeboards were not installed on all edges of tubular welded frame scaffold where employees were exposed to falling objects. Tools and materials were stored thoroughout the 2nd, 3rd and 4th level of the scaffold.
