29 CFR 1910.119(o)(4):The employer did not promptly determine and document an appropriate response to each of the findings of the compliance audit, and document that deficiencies have been corrected: Cargill, Inc. DBA Cargill Meat Solutions Corporation, 1505 East Burlington Avenue Fort Morgan, CO 80701: On or about February 10, 2025, the employer did not correct known deficiencies in equipment that were identified in the Process Safety Management Mechanical Integrity Independent Visual Inspection conducted between March 28, 2023 and April 15, 2023. This exposed employees to inhalation hazards associated with ammonia. The deficient equipment includes but is not limited to the following: (a) Compressor LS-4: improperly terminated insulation end on suction piping; (b) Flash compressor: failed and damaged coating and visible corrosion on the compressor suction valves; (c) Compressor HS-1: coating failure and significant visible corrosion on the AUX OIL COOLER SHELL; (d) Compressor HS-4: damaged insulation end cap at compressor suction valve; (e) Compressor HS-5: oil leak from the compressor slide valve controls; (f) Compressor HS-6: missing the insulation end cap at compressor suction valve; (g) Compressor HS-7: visible oil leaks from the compressor slide valve controls and missing the insulation end cap at compressor suction valve; (h) West auto purger: damaged insulation; (i) Condenser 1: no data plate, visible corrosion, and a damaged fan shroud on the middle fan unit; (j) Condenser 5: visible corrosion on the coil-side condenser and through-frame corrosion on the fan side of the unit; (k) Condenser 6: no functional pressure gauges; (l) Condenser 7: cabinet seal leaks; (m) Condenser 8: piping support is twisted between relief discharges; (n) Condenser 9: significant cabinet seal leaks and significant corrosion and pitting on the HSD piping; (o) Oil Cooler HS-5: oil leaking from the HX PRV and/or discharge piping connections to oil separator; (p) Turbo Chiller: excessive icing on quilt plates and manifold; (q) Blood plasma chiller: oil drain is not properly plugged and uncoated ammonia and relief piping with visible corrosion behind unit; (r) Triple chiller: oil drain is not properly plugged and uncoated piping with visible corrosion; (s) North, south, and middle contherms: uninsulated and uncoated piping with visible corrosion; (t) Rooftop piping: damaged insulation, failed coatings, and significant corrosion and pitting; (u) HS-1 oil separator: multiple areas of failed coating and visible corrosion; (v) HS-4 oil separator: multiple areas of failed coating and visible corrosion; (w) Los side accumulator: total insulation failure on vessel piping and shell; (x) High side Accumulator: significant corrosion visible on exposed, uncoating vessel piping at CPL 97-E and CPL 97-F; (y) CPR: significant corrosion visible on exposed, uncoated piping near HTRL 97-N & HTRS 97-D; (z) Pilot Receiver: failed coatings on vessel shell and associated piping; (aa) North and south contherm surge drums: both vessels have substantial coatings failures and significant corrosion visible on vessel shells and associated piping; (bb) Evaporator 100A: significant corrosion on suspension hardware and through-cabinet corrosion on top of unit; (cc) Evaporator 101A: damaged fan, significant corrosion on suspension hardware and through-cabinet corrosion on top of unit; (dd) Evaporator 100B: damaged fan, excessive icing visible on coils, and total insulation failure on HTRL piping; (ee) Evaporator 27D: total insulation failure on HTRL piping; (ff) Evaporator 26B: missing fan unit; (gg) Evaporator 25A: Significant corrosion visible on unit cabinet; (hh) Evaporator 25C: Significant corrosion visible on unit cabinet; (ii) Evaporator 103A: significant cabinet corrosion on top of unit.
