29 CFR 1910.178(m)(6): A safe distance was not being maintained from the edge of the ramp. On or about August 14th, 2024, employees engaged in material handling operations in the commercial bakery facility located at 4651 F Street, Omaha, Nebraska 68117, were exposed to struck-by/crushing and fall hazards in that a safe distance was not maintained from the edge of the docking ramp on door #2. As a result, an employee backed his forklift off the docking ramp and the forklift fell over four feet impacting the concrete below.
