29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a)Protective Coating Specialists, Inc. at 1350 7th Street Northwest, Huron, SD 57350: On or about and at times prior to November 22, 2023, the employer did not ensure employees were protected from caught-in hazards when operating the Advanced Hydraulics Inc. 25 Ton Hydraulic Press in that one of the two hand controls were bypassed allowing employee exposure to the point of operation during use. This condition exposed employees to amputation and caught-in hazards.
