29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. Employees engaged in distribution operations in the Main Shipping Area located at 4501 South 36th Street, Omaha, Nebraska 68107, were exposed to electrical hazards in that the employees utilizing the electrical switch for dock leveler number 2 were exposed to live parts. The door was fabricated in house to replace the manufacturers original equipment and did not have any listing or labeling.
