29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy: On or about January 26, 2023, to January 27, 2023, at an establishment located at 2099 Badgerland Drive in Green Bay, WI, in the Effluent Room; a junction box used to power the high and low hide pump floats had the hinged cover in the open position with the live conductors protruding out of the box, exposing employees to potential electrical shock hazards.
