29 CFR 1910.134(h)(4)(i): Respirators that failed an inspection or were otherwise found to be defective were not removed from service and discarded or repaired only by persons appropriately trained to perform such operations. a. June 16, 2021, at the top coat powder spray booth, the employer does not ensure that a supplied air respirator (SAR) with tape at the respirator visor was removed from service and discarded or repaired. Employees were required to wear supplied air respirators during top coat powder spraying operations and were exposed to respiratory hazards including, but not limited to 1,3,5-triglycidyl isocyanurate (TGIC), respirable dust, and total dust. b. June 24, 2021, at the top coat powder spray booth, the employer does not ensure that a supplied air respirator (SAR) with tape at the respirator visor was removed from service and discarded or repaired.
