29 CFR 1910.119(l)(4): Process safety information was not updated when a process change covered by 29 CFR 1910.119(l) occurred: On or about February 27, 2021 workers in the Lion Oil Refinery in El Dorado, AR, were exposed to hazards that impact safety when a change to a process covered by 29 CFR 1910.119(l) was made without updating process safety information. (a) In Unit 6, piping was added to the bottom of the Separator Column, but the process safety information, including but not limited to Process Flow Diagrams, Piping and Instrumentation Diagrams, and Piping Specifications were not updated. (b) In Unit 9, a change to Naphtha Piping was made to allow for connection directly to the Unit 6 Separator Column but the process safety information, including but not limited to Process Flow Diagrams, Piping and Instrumentation Diagrams, and Piping Specifications were not updated.
