29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures were not permanent, continuous, and effective: a) Deli Storage Area, on or about 6/06/19: Gilbert Bug Zapper did not have a permanent, continuous and effective path to ground in that the ground pin was missing from the plug connector. b) Produce Cooler Area, on or about 6/06/09: The path to ground from circuits was not permanent, continuous and effective in that the electrical conduit leading to the light switch had voltage present. c) Shipping/Receiving Area, om or about 6/06/19: The path to ground from circuits was not permanent, continuous and effective in that the electrical conduit leading to the light switch had voltage present.
