29 CFR 1926.502(i)(4): All covers were not color coded or they shall be marked with the word "HOLE" or "COVER" to provide warning of the hazard: Note: This provision does not apply to cast iron manhole covers or steel grates used on streets or roadways. location: 3rd floor, northwest corner stairwell B on or about: 11/28/18 a) Employees were working beside and on top of pieces of plywood, supported by planks that were covering the L shaped stairwell opening. The plywood covers of the stairwell opening were not color coded or marked, exposing the employees to a fall of up to 13'8". NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
