29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a. On or about February 28, 2018, located in the Mill Room, employee performed setup activities on the wig wag barrel and wig wag puller without being authorized to perform a lockout/tagout activity, thereby exposing the employees to a caught-in hazard. b. On or about March 8, 2018, located in the Mandrel Hose "Geese" Finishing Department, employee performed the sharpening of blades on water-supplied swing saws without being authorized to perform a lockout/tagout activity, thereby exposing the employees to the rotating blade on the saw. c. On or about March 14, 2018, located in the Mandrel Hose "Geese" Finishing Department, employee performed blade changes on water-supplied swing saws without being authorized to perform a lockout/tagout activity, thereby exposing the employees to rotating parts.
