29 CFR 1926.1207(c): The training must establish employee proficiency in the duties required by this standard and must introduce new or revised procedures, as necessary, for compliance with this standard. On July 7, 2017, at the Wet Well A-16, the employer did not ensure employees understood their roles and responsibilities as authorized attendant, authorized entrant and authorized supervisor. Employees working in the confined space were exposed to asphyxiation and drowning hazards.
