29 CFR 1910.119(j)(4)(iv): The employer did not document the date of inspection, the name of the person performing the inspection, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection, and the results of the inspection for each piece of process equipment. a.) KC Midstream Solutions, LLC, 101 Queen Creek Rd, Endeavor, PA - On or about January 11, 2017, and at times prior thereto: employees were potentially exposed to fire and/or explosion hazards in that, the employer had not documented inspections and tests performed on process equipment including, but no limited to, pressure relief valves, 200,000 BTU fired heater, the 30,000 gallon hydrocarbon storage tank, or associated piping.
