Presque Isle Wine Cellars, Inc., 9440 West Main Street, North East, PA 16428, press pad area, on or about September 15, 2016 and times prior thereto: A slip-on style guard was placed over a rotating shaft end of an auger used to convey grapes from a hopper to a crusher machine. The guard was not affixed so that the guard could not be freely removed while the shaft end is rotating.
