29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: Employees performing welding activities are exposed to electrical shocks and burns when using the Millermatic 252 MIG welder and the employer did not ensure that damaged equipment was removed from service. This violation was last observed on September 6, 2016, in the Fabrication Shop. To abate this violation, the employer must ensure that all damaged equipment is removed from service. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a copy of site-specific program detailing the procedures used to ensure compliance and training of employees.
