29 CFR 1910.253(e)(5)(iv): Hose connections were not clamped or otherwise securely fastened in a manner that would withstand, without leakage, twice the pressure to which they would normally be subjected in service, but in no case less than a pressure of 300 psi: (a) Maintenance Shop: On or about March 9, 2016, and times prior, oxygen and acetylene burning torches, that had been repaired, were not pressure tested before being used.
