29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a.) Edge Plastics, Inc. located at 449 Newman St. Mansfield, OH 44902; On or about December 18, 2015, the employer did not assure employees utilized electrical safety related work practices while verifying zero energy on plastic injection molding press #16. Employees were required to perform work on 480V equipment and were exposed to electric shock and arc flash hazards.
