29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) On or about 05/06/2015, a re-locatable power strip located in C-Building, Metal Cut Saw area, was used in lieu of permanent wiring. A portable radio was plugged into it. (b) On or about 06/02/2015, the switch(es) used for turning on and off the Adams 3 speed flame was not designed to prevent unintentional restarting of the machinery and/or constructed and mounted to minimize inadvertent operation.
