29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: On or about October 8, 2014, in the Blast Department, airborne exposures to silica, crystalline quartz (respirable) were not evaluated for employee(s) who were wearing Bullard continuous flow supplied air respirators during abrasive blasting operation on rough castings in the blast booth and Saf-T-Fit N95 filtering facepiece respirators for processes associated with blasting operations, including dry sweeping and shoveling blasting media in the booth, reloading the blasting hopper with the sweepings, moving castings in and out of the booth with a forklift, and shoveling out the dust collection box for the bag house, exposing employees to the inhalation hazards of silica.
