29 CFR 1910.111(b)(10)(iv): Each vehicle transporting ammonia in bulk except farm applicator vehicles was not equipped with a full face mask. Full face mask respirators for usage in ammonia containing atmospheres were not available for employee bulk transport drivers on their trucks for the ammonia release on March 20, 2014. An employee driver unloading transported anhydrous ammonia at the Midwest Farmers Cooperative facility in Tecumseh, NE, suffered fatal injuries resulting from ammonia exposure and an appropriate respirator was not available on his truck.
