29 CFR 1926.1419(g): If the operator becomes aware of a safety problem and needs to communicate with the signal person, the operator must safely stop operations. Operations must not resume until the operator and signal person agree that the problem has been resolved. a. Located at the site, there were employees removing a 60 foot, 3 inch I-beam from a bridge with a Bucyrus Erie 60 ton lattice boom crawler crane, model 30B, serial number 134548 that was rigged 6 foot, 4 inches off of the east side of the I-beam's center and 18 feet, 8-1/2 inches of the west side of the I-beam's center. The crane operator identified it as safety problem and did not safely stop operations, thereby exposing the employees to a struck-by hazard.
