Text For Citation: 01 Item/Group: 007A Hazard: CUTS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard: a) Production area, front room - A minimum safe distance program was not implemented for when the hands of the operators of a Pacific Hydraulic Presses and Shears hydraulic press brake (model J135-12, serial number 6681) came within four inches of the point of operation. On or about 7/21/11. A feasible means of abatement would be to establish and implement a minimum safe distance program that conforms to the requirements of ANSI B11.3-1982 paragraph 6.1.4.3 and contains the training elements listed in OSHA Directive CPL 2-1.25. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
