29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: On or about August 1, 2025, and at times prior, on a site located at 4529 Hypoluxo Road, Lake Worth, Florida, the employer did not complete an entry permit as required, to document safe permit space entry operations for employees exposed to hazardous chemical vapors such as, but not limited to Benzene, Toluene and Merican 9505 Unsaturated Isophthalic Resin when entering an underground fuel tank to perform a repair.
