29 CFR 1910.146(k)(2)(i): The employer did not ensure that each member of the rescue service was provided with, and trained to use properly the personal protective equipment and rescue equipment necessary for making rescues from permit spaces: On or about June 16, 2025, employees were exposed to hazardous atmospheres while working in railcars and the employer did not ensure each prospective rescuer was provided a retrieval line and respiratory protection necessary for IDLH atmospheres.
